MOU – Baca County DHS

Memorandum of Understanding (MOU) between
Baca County Public Health Agency (BCPHA)
Baca County Department of Human Services (DHS)
This Memorandum of Understanding (MOU) is entered into by the Baca County Public Health Agency and the Baca County Department of Human Services for the purpose of sharing information of and on shared cliental and having a working agreement to uphold HIPPA at all times with and between both agencies.
LIFE OF THIS AGREEMENT
This agreement is effective on the 2nd day of January, 2025, and terminates upon mutual agreement between the Baca County Public Health Agency and Baca County Department of Human Services.
PURPOSE OF THIS AGREEMENT
The purpose of this agreement is that Baca County Public Health Agency and Baca County Department of Human Services can share information of and on shared cliental between the two agencies. The information can only be shared if an ‘Authorization for Release of Information’ form has been completed and signed. HIPPA guidelines must be followed at all times and in all requests/situations resulting in information sharing. No information will be shared unless the proper document listed has been completed.
