The Community Service Block Grant (CSBG) Program assists Baca County residents in need of medical assistance such as vision, dental, hearing aids, or other qualifying needs. The CSBG Program also assists Baca County residents with emergency funding assistance with utility shut-off notices, emergency food needs, and long distance travel for medical appointments. The CSBG Program is available to all eligible Baca County residents regardless of race, color, national origin, sex, disability, or sexual orientation.
Medical Assistance will pay up to $400.00 per person. The funding is available per person per year excluding eye glasses and corrective lenses, which is limited to every two years.
Emergency funding is limited to one time per year.
• Utility Shut-Off Notice: receive assistance up to $250.00 onetime per year
• Food Needs: receive up to $150.00 per year in increments of $50.00 twice within 6 months
• Transportation for Long Distance Medical Appointments: $150.00 per year
Qualifications are measured by the CSBG Program requirements. Currently the Federal guidelines are at 200% of the FPL this applies to all service providers using the CSBG funding. For more information or to schedule an appointment to complete the application process, please call 719.523.6621.
