Community Health Assessment (CHA)
BCPHA recently completed the Public Health Improvement Plan for 2025-2029. This plan contains various health data points that determine where health problems exist and what contributes to those problems. The CHA includes a review of various health indicators that were collected using existing data sets, subject matter expert surveys, and existing resource assessments. Community input and participation were essential to ensure the process was community driven, and results reflected the needs and perspectives of county residents. Combining data and trends with input from local partners, the CHA leads into the development of the Public Health Improvement Plan (PHIP), a community driven plan which identifies key strategies to address the health concerns identified in the CHA.
